A deep plane facelift is a procedure that reduces the appearance of facial aging by repositioning the deeper layers of the facial tissue, creating a lifted and more youthful-looking appearance. As you age, your body’s levels of collagen and elastin naturally decline, leading to skin and muscle drooping. During the deep plane facelift, Dr. Miotto shapes the facial and neck tissues to diminish the signs of facial aging.

About Deep Plane Facelifts
A deep plane facelift is a procedure that rejuvenates your appearance by shaping the drooping tissue, removing excess skin, and targeting the deeper layers of the face. During a deep plane facelift, Dr. Miotto will work in the deeper layers of the face, the sub-SMAS plane, and will reposition the facial structures to provide long-lasting and natural-looking facial rejuvenation. (1) If you have moderate to severe skin laxity, a deep plane facelift can help you enhance your appearance without the risk of a “pulled” or unnatural look.
Why The Deep Plane Facelift?
Deep plane facelifts have become increasingly popular in recent years because they address the root causes of facial aging more comprehensively than other techniques, especially in the midface and jawline. Unlike more limited techniques that pull and tighten only the skin or superficial layers, the deep plane technique releases and repositions the facial ligaments, facial fat, and muscles. Because the deeper structures are lifted rather than just tightened, results often last longer, though this depends on genetics, lifestyle, and skin quality. Dr. Miotto dedicates her efforts to not only delivering a great surgical result, but she is also constantly refining her techniques so patients experience better healing and a decreased recovery.
Benefits
There are several benefits to a deep plane facelift, which include:
- Customizable treatment: Each procedure is tailored to your unique anatomy, ensuring that results appear natural and harmonious with your features.
- Increased confidence: Patients report a very positive impact they see in themselves after the deep plane facelift and a great boost in their confidence.
- Easily hidden incisions: Incisions are carefully placed in natural facial creases and folds to ensure minimal scarring.
- Smoother skin: The deep plane facelift effectively removes excess skin and creates a smoother skin appearance.
- Lasting results: By addressing the skin, fat, and muscular structures, the results from a deep plane facelift can last for several years.
- Expert care: Deep plane facelifts are the only facelift technique Dr. Miotto performs, allowing patients to benefit from her expertise and refined techniques. For some patients, it is possible to offer a “baby” version of the procedure if the neck skin does not present excessive aging.
Candidates for a Deep Plane Facelift
A deep plane facelift will effectively address sagging skin and visible jowls. Candidates seeking these corrections should be in good general health and be prepared to take good care of their health after treatment. Being healthy and engaged in skin health protection is important before and after surgery. Patients should also keep realistic expectations regarding the results of their deep plane facelift. Depending on your level of skin laxity, Dr. Miotto may be able to address your concerns with nonsurgical alternatives and will guide you with a long-term treatment plan for your overall skin health. (2) During your consultation, your candidacy for a deep plane facelift or another procedure will be further discussed.
Personal Consultation
During your consultation, it is important to have a candid conversation with Dr. Miotto about your aesthetic goals to determine whether or not you are a good candidate for her deep plane facelift. She will discuss the results you are looking to achieve, your aesthetic concerns, and any questions you have. Additionally, Dr. Miotto will perform a physical examination. Your history of previous surgeries, previous fillers, lasers, and energy device treatments is important to disclose. With all the necessary information collected, she will create a treatment plan that caters to your anatomy and appearance goals.

Preparation
Dr. Miotto will provide you with important preparation instructions to follow before your procedure. Dr. Miotto does not operate on patients who smoke. Patients who smoke will need to quit several months in advance, and ideally, forever. Smoking will make you age faster and with more health problems, which will definitely affect your appearance and results. All patients should avoid alcohol for the week before and two weeks after surgery, as well as medications or supplements with blood-thinning properties. We recommend that you fill the prescriptions recommended by Dr. Miotto ahead of your surgery day, and that you schedule time off work to be able to heal. Dr. Miotto will let you know how much time to schedule off work based on your specific treatment plan. You should also arrange for someone you trust to care for you for several days after your deep plane facelift surgery. Having a good supporting system is so important to help you heal and recover fast.
Procedure
On the day of your procedure, you should arrive with clean skin, without makeup or skincare products. Your caregiver must accompany you to the surgery center and be available on the day of your surgery. After anesthesia is administered, Dr. Miotto will inject a local anesthetic into your face, so everything is numb before she starts her surgery, even if you are under deep sedation or general anesthesia. That reduces the need for strong general anesthesia, reduces postoperative pain and nausea, and helps reduce bleeding in surgery. Dr. Miotto will carefully make the necessary adjustments, including repositioning fat, tightening muscles, removing excess skin, and more. After the desired results are achieved, your incisions will be closed with very fine sutures, and your head will be wrapped gently with soft pads and bandages (which stay in place for 24 hours). You will stay in the recovery room, where the surgery center team will monitor you until you are ready for discharge.
After Your Surgery
After your monitoring period, you can return home or to your hotel to rest and recover. You should have a trustworthy adult caregiver to drive you home afterward and care for you during your initial recovery. You will be instructed about your next day follow-up appointment.
Most patients can return to work and resume their normal routines after approximately 2 weeks. Patients typically see a decreased recovery time from Dr. Miotto’s refined deep plane facelift techniques and preoperative preparation, allowing them to return to their normal routines sooner than with traditional methods.
Results
After undergoing a deep plane facelift, you can enjoy a lifted, refreshed look and a more youthful-looking appearance. Jowls and other droopy facial skin will be effectively addressed. Because Dr. Miotto makes adjustments to deep structural tissues, the results of your deep plane facelift are long-lasting. However, we do not stop aging with surgery; it will have its normal course after surgery. Surgery is a “reset” of the aging that has happened. Proper skincare, nutrition, sleep, hormonal balance, and avoidance of excessive sun exposure will be important for prolonging your results.
Corresponding Treatments
Facial Fat Transfer
A facial fat transfer is a treatment that uses your own fat to enhance your appearance. If you would like to address facial volume loss, Dr. Miotto can remove excess fat from the thighs and/or abdomen and inject it into the face to help you achieve a smoother, plumper, and more youthful-looking appearance. Dr. Miotto uses the most advanced fat harvest and preparation – microfat and nanofat – to improve both facial volumization and skin quality.
Neck Lift
Facelifts and neck lifts are almost always performed during the same surgery day because there is usually facial and neck aging happening at the same time. Both the face and neck age together, so addressing one without the other would likely fail to provide substantial rejuvenation. A neck lift removes droopy skin and tightens the laxity in the muscles of the neck to rejuvenate your appearance, especially the jawline. This procedure also improves muscle banding and deep neck volume of genetic necks.
